CMS enrollment and 2024 claims rows join only by exact CCN; reported owners link through exact CMS enrollment IDs. Ownership and claims fields are descriptive context, not quality measures or patient prices. Exact same-type identity and office signals link Illinois licenses to CMS records; unmatched licenses remain state-only. Provider ZIP codes are joined to 2025 Census ZCTA centroids, with labeled fallbacks where needed. Points represent approximate office areas, not exact offices or service areas.
