How Care Record assembles Medicare evidence
This release maps CMS hospice and home-health agency records by stable certification number and adds current Illinois IDPH license-directory evidence by state license number. CMS enrollment and 2024 claims rows join only by exact CCN; reported owners link through exact CMS enrollment IDs. Ownership and claims fields are descriptive context, not quality measures or patient prices. Exact same-type identity and office signals link Illinois licenses to CMS records; unmatched licenses remain state-only. Provider ZIP codes are joined to 2025 Census ZCTA centroids, with labeled fallbacks where needed. Points represent approximate office areas, not exact offices or service areas.
Record completeness
Record completeness reports how many core identity fields are populated in the CMS row. It is not a clinical quality, safety, availability, price, or suitability score. Ownership, certification age, and claims payments are descriptive only.
Merge rules
Each source maps its native columns into canonical fields. Lower priority numbers win when two sources provide different values. Every observation remains attached to the record with its source URL and observation date.

Pricing evidence
Published prices remain separate observations with their billing unit, basis, effective period, check date, and direct source. Market benchmarks are attached at the most specific matching geography and are not treated as provider quotes.

Limitations
CMS certification and Illinois state licensure are separate evidence. Neither source guarantees current availability, service area, suitability, or care quality. Separately displayed CMS submitted-charge and Medicare-payment averages summarize 2024 Original Medicare fee-for-service claims; they are not provider cash prices, patient out-of-pocket amounts, quotes, or coverage determinations. Map points use ZIP-area centroids rather than exact office coordinates. Verify urgent details directly with the agency, Medicare, and state regulators.
